The plain-English definition
Move the needle means produce a result big enough to notice — borrowed from the gauges and dials of old instruments, where a small change barely twitches the needle and a real change swings it. "Will this move the needle?" = "Will this actually matter?"
The missing dial
Here is the problem: the metaphor assumes there is a dial, and that anyone read it. To know whether something moves the needle, you first need a baseline — a number you measured before. Most teams that ask "does this move the needle?" have never measured where the needle currently points. So the phrase becomes a polite way of saying "I don't think this matters," dressed up as a rigorous question.
It also lets the asker stay vague. "Move the needle on what?" is the unasked follow-up. Without a defined metric, nothing ever moves the needle, because there is no needle.

Is "move the needle" a red flag?
Low as fluff, higher as a filter. When a leader keeps asking "does this move the needle?" without ever defining the needle, the phrase is doing the work of "no" while sounding like "let's be rigorous." It is also a way to look demanding without committing to a target. The asker sounds like they want big results, but they have handed you a standard no one wrote down — so they can reject quietly later and call it rigor. Notice, too, that the work being dismissed is rarely theirs.
